Snowline Hospice of El Dorado in California is seeking a dedicated Full-time Home Health Aide who shares our mission to deliver compassionate, patient-centered care in home settings. Qualifications

• Certified Nursing Assistant (CNA) certification required.
• Certified Home Health Aide (CHHA) preferred.
• Current BLS/CPR certification.
• One year aide experience; hospice experience preferred.
• Valid California driver’s license and access to reliable vehicle.
• Strong observation and communication skills.
• Experience with electronic medical records (EMR) preferred.
• Flexible schedule including evenings or weekends.
• Reliable transportation within service area.

Responsibilities

• Coordinate and provide care to hospice patients.
• Calm, soothe and empathize with patients and families.
• Collaborate with healthcare team in delivering high-quality care.
• Address personal care needs and promote comfort.
• Document care and observe patient condition.
• Participate in interdisciplinary meetings and training.

• Market Benchmarked Salaries: Salary: $22.00-$30.00 per hour

Responsibilities

• To coordinate and provide care to our hospice patients
• To calm, soothe and empathize with the strong emotions our patients and families are experiencing
• To participate with a team of healthcare professionals in superior level of care for each patient
• To address personal care needs and promote patients’ comfort needs
• Collaborate on cases, involvement in interdisciplinary meetings, training and development and other organizational activities
• Assist with observing the patient along with documenting care

Requirements

• Certified Home Health Aide issued by the State Department of Public Health preferred and
• Certified Nursing Assistant Certificate issued by the State Department of Public Health required
• Current BLS/CPR certification
• One year aide experience required and one year of hospice experience preferred
• Possession of a valid California Driver's License, access to a reliable automobile and minimum car insurance coverage
• Excellent observation, problem solving, verbal and written communication skills
• Demonstrated ability to efficiently use a computer, prior electronic medical record (EMR) experience preferred
• Open to an occasional flexible schedule to work evenings or weekends
• Reliable transportation and comfort traveling throughout service area
