Job summary

Affinity Hospice Care Inc. is seeking a Certified Home Health Aide to provide compassionate, hands-on support in patients’ homes. You will work under a Registered Nurse to assist with daily personal care and safety, improving quality of life.

The role is per diem with a visit rate of $35-$45. Requirements include a California CHHA certificate, CPR, a valid driver’s license, and the ability to work independently with empathy and professionalism.

Qualifications

• California Certified Home Health Aide certificate required.
• Current CPR certification with AHA.
• Valid driver’s license and insured vehicle.
• At least 18 years of age.
• Able to read, follow written instructions, and document care accurately.

Responsibilities

• Provide personal care including bathing, grooming, oral hygiene, dressing, and skin care.
• Assist with mobility, exercises, and range-of-motion activities per the care plan.
• Support patients with meals—from preparation to feeding assistance.
• Take and record vital signs as directed.
• Help patients with medication self-administration.
• Keep the patient’s living space clean, safe, and orderly.
• Observe patients and report changes to the supervising nurse.
• Document care provided according to organizational standards.
• Perform other duties as required for patient services.

• Comfortable working independently with minimal supervision
