Qualifications
- Certified Nursing Assistant (CNA) certification required.
- Certified Home Health Aide (CHHA) preferred.
- Current BLS/CPR certification.
- One year aide experience; hospice experience preferred.
- Valid California driver’s license and access to reliable vehicle.
- Strong observation and communication skills.
- Experience with electronic medical records (EMR) preferred.
- Flexible schedule including evenings or weekends.
- Reliable transportation within service area.
Responsibilities
- Coordinate and provide care to hospice patients.
- Calm, soothe and empathize with patients and families.
- Collaborate with healthcare team in delivering high-quality care.
- Address personal care needs and promote comfort.
- Document care and observe patient condition.
- Participate in interdisciplinary meetings and training.
