Job summary

Clermont Senior Services Inc. seeks a compassionate Home Health Aide to deliver personal care and homemaking services in clients' homes across Ohio. You will support licensed nurses and case managers, assisting with bath, grooming, mobility, nutrition, and medications while maintaining confidentiality and following agency procedures.

The role requires a high school diploma or GED, STNA or Home Care Aide program completion, and competency evaluation. Training and supervision are provided.

Qualifications

• High School diploma or GED.
• Certified State Tested Nurse Aide (S.T.N.A.) in Ohio.
• Graduate of an approved Home Care Aide Program.
• One year of supervised direct service delivery in health care in the last three years.
• Nursing students who can verify successful completion of Fundamentals of Nursing Care.
• Successful completion of a competency evaluation (written exam and skills demo).
• Meet the agency's general requirements.

Responsibilities

• Provide personal care services (bed bath, grooming, oral hygiene) as assigned, with safe transfer and mobility.
• Offer homemaking tasks (light housekeeping, laundry, meal prep, errands).
• Deliver respite services and companionship as directed by professionals.
• Serve as liaison among the customer, Home Care Manager, and case manager; report risks promptly.
• Accurately complete and submit required documentation within deadlines.
• Attend scheduled staff meetings and training; perform other duties as assigned.

SPECIFIC RESPONSIBILITIES:

• Responsible for ensuring no confidential information is discussed or shared unless it is in the course of conducting valid Agency business.
• Responsible for conducting work in conformance with Agency’s policies and procedures in accordance with federal, state, and local laws and regulations
• Responsible for providing appropriate and safe personal care services as assigned to include bed bath, sponge, tub or shower bath, shampoo (sink, tub, or bed), nail and skin care, oral hygiene, toileting and elimination, safe transfer techniques and ambulation, normal range of motion and positioning, adequate nutrition and fluid intake, and any other task assigned by the licensed nurse, including special tasks requiring instruction by the licensed nurse and return demonstration by the home health aide.
• Responsible for providing appropriate and safe homemaking services as assigned by the nurse or social service case manager to include light housekeeping, laundry, meal preparation, and essential errands and grocery shopping.
• Responsible for providing appropriate and safe respite services, including personal care, homemaking, and companionship as assigned by the appropriate professional.
• Act as a liaison between the customer, the Home Care Manager, and case manager, including prompt reporting of customers at immediate risk or endangerment.
• Responsible for accurately completing and submitting all required customer or Agency documentation within required time frames.
• Attend and participate in all scheduled staff meetings and training.
• Performs other duties and responsibilities as assigned.

Requirements

BASIC QUALIFICATIONS:

• High School diploma or GED OR
• Certified State Tested Nurse Aide (S.T.N.A.) as verified by Ohio Nurse Aide Registry. OR
• Graduate of an approved Home Care Aide Program, without a 24-month lapse in employment since completing program. OR
• One year of supervised institutional or community based direct service delivery in healthcare within the last three years. OR
• Nursing students who can verify the successful completion of the Fundamentals of Nursing Care.
• Successfully complete a competency evaluation including a written exam and demonstration of basic skills.
• Must meet the Agency’s general requirements.
