Job summary

Clermont Senior Services Inc. in Batavia Township, OH is seeking a Home Health Aide to support seniors with daily living activities. You will assist with eating, bathing, transferring, and ambulating, and help with scheduled activities and crafts under supervision.

The role includes providing safe companionship and reporting any risks to the RN Manager. You will ensure compliance with Agency policies, maintain confidentiality, and prepare the work area for the next day, while following CACFP

Qualifications

• High School diploma or GED.
• Certified State Tested Nurse Aide (S.T.N.A.) as verified by Ohio Nurse Aide Registry.
• Graduate of an approved Home Care Aide Program, without a 24-month lapse in employment since completing program.
• One year of supervised institutional or community based direct service delivery in healthcare within the last three years.
• Nursing students who can verify the successful completion of the Fundamentals of Nursing Care.
• Must pass a competency evaluation with a written exam and demonstration of basic skills.

Responsibilities

• exemplify the Service with Heart attitude expected of all employees.
• Ensure confidentiality and compliance with Agency policies, procedures, laws, and regulations.
• Assist with Activities of Daily Living (ADL) such as eating, bathrooming, transferring, and ambulating as appropriate.
• Assist the ADS Activity Specialist with planned activities and crafts.
• Provide safe respite services including personal care and companionship. Help customers board and alight from the bus.
• Serve as liaison between customers and RN Manager with prompt reporting of risk or endangerment.
• Complete and submit required Agency documentation on time.
• Prepare the ADS area for the next day.
• Follow CACFP and other funding guidelines for food and beverages.

Education

High School diploma or GED

Certified State Tested Nurse Aide (S.T.N.A.)

Graduate of an approved Home Care Aide Program

One year of supervised direct service delivery in healthcare

Nursing students who can verify Fundamentals of Nursing Care

SPECIFIC RESPONSIBILITIES:

• Will exemplify the, Service with Heart attitude that is expected of all employees.
• Responsible for ensuring no confidential information is discussed or shared unless it is in the course of conducting valid Agency business.
• Responsible for conducting work in conformance with Agency's policies and procedures in accordance with federal, state, and local laws and regulations
• Responsible to help with Activities of Daily Living Skills (ADL's) to include: eating, bath rooming, transferring, ambulating, etc., as appropriate.
• Responsible for assisting the ADS Activity Specialist with planned activities and crafts.
• Responsible for providing appropriate and safe respite services including personal care and companionship as assigned by the appropriate professional. Assist customers to/from the bus.
• Provide warm, hospitable atmosphere for customers where they feel comfortable attending and participating, and are not afraid to seek out help when needed.
• Responsible for acting as a liaison between the customer and the RN Manager. This will include the prompt reporting of customers at immediate risk or endangerment.
• Responsible for accurately completing and submitting all required Agency documentation within the required time frames.
• Responsible for seeing that the ADS area is prepared for the next day.
• Responsible to follow the CACFP and other funding source guidelines to prepare, heat and serve food and beverages.
• Complete Home Health Aide orientation for cross-training purposes.
• Attend and participate in all scheduled staff meetings and training sessions.
• Performs other duties and responsibilities as assigned.

Requirements

BASIC QUALIFICATIONS:

• High School diploma or GED OR
• Certified State Tested Nurse Aide (S.T.N.A.) as verified by Ohio Nurse Aide Registry. OR
• Graduate of an approved Home Care Aide Program, without a 24-month lapse in employment since completing program. OR
• One year of supervised institutional or community based direct service delivery in healthcare within the last three years. OR
