Primehhcc in East Longmeadow, MA is seeking a compassionate Home Health Aide (HHA) / Certified Nursing Assistant (CNA) to provide client-centered personal care at clients' homes under supervision.

The role supports activities of daily living, monitors vital signs, assists with meals, light housekeeping, transportation, and documentation using GPS/EVV; requires MA HHA/CNA license and CPR.

Qualifications

• HHA certification (MA) or CNA license.
• CPR certification required.
• 1 year home care experience preferred.
• Reliable transportation and willingness to travel within the service area.
• Ability to lift up to 30 pounds and stand/walk for extended periods.

Responsabilités

• Assist clients with activities of daily living (ADLs) including bathing, dressing, grooming, toileting, ambulation and feeding.
• Provide assistance with prescribed exercises and mobility.
• Take and record vital signs; report changes to Nursing Supervisor.
• Provide meals and assist with feeding; light housekeeping.
• Provide companionship and emotional support; escort to appointments.
• Document care accurately and communicate schedule needs using GPS/EVV and company software.

Job Title: Home Health Aide (HHA) / Certified Nursing Assistant (CNA)Area: East LongmeadowReports To: Scheduling Coordinator and Nursing SupervisorRISK OF EXPOSURE TO BLOODBORNE PATHOGENS-HIGHApproximately 19 hours currently availableAdvise as 15–20 hours per weekPosition Summary:The Home Health Aide (HHA) / Certified Nursing Assistant (CNA) provides compassionate, client-centered personal care and supportive services to clients in their homes. Working under the supervision of the Nursing Supervisor and the direction of the Scheduling Coordinator, the HHA/CNA helps maintain the client’s comfort, safety, and dignity while promoting independence.Key Responsibilities:Client Care:Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, ambulation, and feeding.Provide assistance with prescribed exercises and mobility (as trained and permitted by regulation).Provide medication reminders as directed by nursing supervisor.Take and record vital signs, observe and report changes in client condition to the Nursing Supervisor as directed.Prepare meals and assist with feeding as needed.Perform light housekeeping to support a safe and clean home environment.Provide companionship and emotional support.Documentation & Communication:Accurately document care provided and report changes in condition or concerns promptly.Use GPS/EVV app and company software for clocking in and out of shifts.Communicate regularly with the Scheduling Coordinator regarding assignments, availability, and scheduling needs.Follow the individualized care plan as directed by nursing staff.Supportive Services:Provide companionship, socialization, and emotional support to clients.Assist with meal planning and preparation according to dietary guidelines.Perform light housekeeping duties such as laundry, dishwashing, vacuuming, dusting, and general tidying.Assist with errands, grocery shopping, and escort to appointments or social activities (as applicable).Provide reminders for medications(without administering), appointments, and hydration.Client Safety & Communication:Observe and report changes in the client’s condition or home environment to the Nursing Supervisor.Communicate effectively with the Scheduling Coordinator regarding schedule changes, availability, and client concerns.Ensure the client’s living environment is safe and free from hazards.Compliance & Conduct:Adhere to agency policies, procedures, and applicable state regulations.Maintain client confidentiality in accordance with HIPAA guidelines.Attend required training sessions and in-service education.Maintain professional boundaries and uphold the agency's code of conduct.Qualifications:High School Diploma or equivalent (preferred).Current Home Health Aide certification or Certified Nursing Assistant license in Massachusetts.CPR certification (current and valid).One (1) year of experience in a home care setting preferred.Excellent interpersonal skills and ability to work independently.Reliable transportation and willingness to travel within the service area.Physical Requirements:Ability to stand, walk, bend, and lift for extended periods.On occasion, may be required to bend, stoop, reach and move client weight up to 250 pounds; lift/carry up to 30 pounds.Work Schedule:Flexible schedule; may include weekdays, weekends, evenings, and holidays depending on client needs and agency requirements.
