Residential Home Health and Residential Hospice is seeking an Admission RN to provide skilled nursing care to patients in their homes in the Mount Carmel, PA area. A $10,000 first-year bonus is offered for achieving key milestones.

The role involves comprehensive start-of-care assessments, patient and family education, and coordination of admissions with an interdisciplinary team. Travel to patient homes is required.

Qualifications

• Active RN license in state of work.
• Minimum 1 year RN experience in home health or similar setting.
• Valid driver's license and ability to travel to patient homes.

Responsibilities

• Deliver skilled, patient-centered care in home health settings with an interdisciplinary team.
• Conduct start-of-care assessments, including clinical evaluations, med reconciliation, and initial care plans.
• Educate and guide patients and families through admission process and care expectations.
• Apply clinical judgment to coordinate admissions, address barriers, and ensure compliant EMR documentation.
• Initiate evaluations to other disciplines when indicated.
• Work independently, but collaborate with the team as needed.
• Travel to patient homes within assigned territory.

We are hiring an Admission Registered Nurse (RN) for a Home Health position providing skilled nursing care to patients in their homes in the Mount Carmel, PA, area. What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

• Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders

• Educate and guide patients and families through the admission process, setting expectations for care, reviewing diagnoses and treatment plans, and ensuring they feel informed and supported from day one

• Apply strong clinical judgment and critical thinking to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation

• Initiate evaluations to other disciplines when indicated

• Work independently, but never alone

• Travel to patient homes within assigned territory

Admission RN Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
