Job summary

Residential Home Health and Hospice is seeking a dedicated Admission Registered Nurse (RN) to provide skilled nursing care in patients' homes across Cass County. A $10,000 first-year success bonus is offered to reward key achievements during the first year.

You will conduct start-of-care assessments, develop care plans, educate patients and families, and coordinate with an interdisciplinary team. Travel to patient homes within the territory is required, with a focus on autonomy and meaningful

Qualifications

• Active Registered Nurse license in the state where you work
• One year of RN experience delivering patient care
• One year of home health experience as an RN, preferably conducting OASIS assessments
• Current driver’s license and ability to drive ~20% of the day to patient locations

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team
• Conduct start-of-care assessments, clinical evaluations, medication reconciliation, and initial care plans
• Educate patients and families through the admission process and set expectations for care
• Apply clinical judgment to coordinate admissions, address barriers to care, and document in EMR accurately and timely
• Initiate evaluations to other disciplines when indicated
• Work independently, but collaborate with team as needed
• Travel to patient homes within the assigned territory

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you

• Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders

• Educate and guide patients and families through the admission process, setting expectations for care, reviewing diagnoses and treatment plans, and ensuring they feel informed and supported from day one

• Apply strong clinical judgment and critical thinking to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation

• Initiate evaluations to other disciplines when indicated

• Work independently, but never alone

• Travel to patient homes within assigned territory

Admission RN Qualifications

• Active Registered Nurse, RN license in the state you work

• One year of RN experience and the clinical expertise to deliver quality patient care

• One year of home health experience as an RN, preferably conducting OASIS assessments.

• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
