Job summary

Iowa Home Care is seeking a Home Health Admission Nurse in West Des Moines to welcome new patients and craft personalized care plans for in-home care. You will collaborate with physicians and the care team to ensure high-quality, compassionate services tailored to each patient’s needs.

Responsibilities include educating families, coordinating with therapists and home health aides, and documenting assessments and updates per company policy.

Qualifications

• Current RN licensure in Iowa.
• Prior experience in home health admission or healthcare admissions is preferred.
• Strong assessment, organizational, and communication skills.
• Valid driver’s license and reliable transportation for travel to client homes.

Responsibilities

• Conduct thorough initial assessments of patients to establish individualized care plans.
• Develop, implement, and adjust care plans based on patient needs and clinical standards.
• Communicate and coordinate with families, therapists, social workers, and HHA staff to ensure a well-informed care team.
• Advocate for patient needs and educate families and caregivers to support the patient’s care.
• Document all assessments, care plans, and updates accurately and timely in accordance with company policies.
