Job summary

Concierge Home Care in Winter Haven, FL is seeking a Home Health Account Executive to grow referrals and admissions across Polk County. You will build trusted partnerships with physicians, hospitals, SNFs, and assisted living communities to drive patient care at home.

The role offers base salary $75,000–$85,000 with uncapped bonuses, autonomy in a growing organization, and strong leadership support. Healthcare professionals transitioning into sales are welcome.

Qualifications

• Healthcare sales experience or strong clinical experience with existing healthcare relationships.
• Excellent communication and relationship-building skills.
• Self-motivated with the ability to work independently.
• Valid driver’s license and reliable transportation.

Responsibilities

• Develop and maintain relationships with physicians, hospitals, SNFs, and other referral sources.
• Increase home health admissions and achieve monthly sales goals.
• Educate referral partners on Concierge Home Care's services.
• Coordinate smooth patient transitions from hospital to home.
• Maintain CRM documentation and referral activity.
• Represent Concierge Home Care at networking events and community outreach opportunities.
