Job summary

NHS Scotland is expanding its Care at Home and Enablement Team in East Ross to support people in their own homes. The role focuses on maximizing independence, preventing hospital admissions, and facilitating safe discharges with a person-centred care plan.

Applicants should be self-motivated, able to work in a team, and communicate effectively within a multi-disciplinary setting. SVQ 2 is preferred or a willingness to pursue it, and a current driving license with own car is essential for field

Qualifications

• Demonstrates self-motivation and dynamic approach to care delivery.

Responsibilities

• Promote independence and enable people to remain at home.
• Help prevent hospital admissions through proactive support.
• Support timely hospital discharges and in-home assessments.
• Contribute to person-centred care plans tailored to individual needs.
• Provide ongoing review to aid transition back to independence and reduce long-term care reliance.
