Job summary

NHS Scotland seeks an Enablement Worker for East Ross, working a 4 days on, 4 days off rotation. You’ll help vulnerable clients remain independent at home, support early intervention, and facilitate safe hospital discharges.

The role involves working in a multi-disciplinary team, with a focus on person-centred care plans and ongoing home visits. A driving license and car are essential; SVQ 2 is desirable. Flexible working may be available.

Qualifications

• Demonstrated ability to work in a multi-disciplinary team.
• Excellent communication and interpersonal skills.
• Willingness to work in home-based care settings.
• SVQ 2 or willingness to obtain it.

Responsibilities

• Maintain independence for vulnerable individuals at home.
• Support prevention of hospital admissions through proactive care.
• Assist with hospital discharge planning and home-based assessments.
• Contribute to person-centred care plans aligned with NHS standards.
• Collaborate with the Care at Home team and oversee ongoing support.
