Job summary

Herewith seeks a compassionate Care Partner to support in-home dialysis during a major care transition. You will provide one-on-one, hands-on dialysis assistance, training will be provided, and you will work with a clinical team to ensure patient comfort and safety.

The role emphasizes patient dignity, clear communication, and reliable care during the transition from clinic to home dialysis, with ongoing clinical support and a stable weekly schedule.

Qualifications

• 1+ year of dialysis experience (home or clinic).
• Willing to complete provided training and work in home setting.
• Strong communication and teamwork with clinical staff.

Responsibilities

• Assist with dialysis treatment setup (NxStage or similar home machines).
• Maintain a clean, organized, safe treatment area.
• Provide companionship and emotional support.
• Help the patient stay on schedule with treatments.
• Offer light ADL support as needed.
• Monitor safety and report concerns to the nurse.
• Maintain shift notes and simple documentation.
