Job summary

Fresenius Medical Care is seeking an RN CAP 2 for Home Therapies at our Boston facility. The role combines care coordination with patient education for Home Dialysis patients and Home Partners.

Requirements include an accredited nursing degree, state RN licensure, CPR/BLS, and at least 2 years nephrology nursing in the last 2 years. Hourly pay ranges from $39 to $65 with a full benefits package.

Qualifications

• Graduate of an accredited School of Nursing.
• Current valid state RN licensure.
• CPR/BLS certification required.
• Minimum 2 years nephrology nursing experience within last 2 years.

Responsibilities

• Provide and coordinate clinically competent care including assessment, planning, intervention and evaluation for assigned patients.
• Monitor dialysis data, adjust treatment plans as needed and document in medical records.
• Educate patient and family; collaborate with home care staff and interdisciplinary team.
• Administer medications per protocol and respond to emergencies as required.
• Lead or precept in unit as directed; participate in quality improvement activities.
