Job summary

Northwest Kidney Centers seeks a full-time day shift Home Program Nurse Care Manager (Home Dialysis Training Nurse) to support the Yesler Kidney Center and other Seattle Kidney Centers in peritoneal dialysis.

The Care Manager will deliver and document care for patients dialyzing at NKC and in their homes, coordinating assessments, planning care, education and training, and ensuring high-quality patient outcomes across care settings.

Qualifications

• Equivalent of Associate or Baccalaureate degree with nursing major.
• Current registered nursing license in the state of Washington.
• One year of previous nursing experience.
• American Heart Association BLS.

Responsibilities

• Deliver and document care for patients dialyzing at Northwest Kidney Centers and in their home.
• Perform assessments, planning care, directing, and evaluating patients and their home environment and clinical results.
• Provide education and training and administer services and programs that enhance patient care.
• Collaborate with the care team to ensure quality patient outcomes.
