Job summary

Herewith is seeking a compassionate Care Partner to support a patient with in-home dialysis in Laguna Niguel, CA. The role offers one-on-one care, with training provided by our clinical team to ensure confidence in home dialysis procedures.

You will assist with dialysis setup, maintain a safe, clean environment, provide companionship, keep to treatment schedules, and document shift notes, contributing to a smooth transition from clinic to home-based care.

Qualifications

• 1+ year of dialysis experience (home or clinic).
• Familiarity with home dialysis systems preferred (NxStage, Tablo is a plus).
• Strong communication and teamwork with clinical staff.

Responsibilities

• Assist with dialysis treatment setup (NxStage or Tablo).
• Keep the treatment environment clean, organized, and safe.
