Job summary

Herewith is seeking a compassionate Care Partner in Laguna Niguel, CA to support an patient transitioning to home dialysis. You will provide one-on-one care, assist with setup, monitor safety, and coordinate with the nurse team while building trust with the client. Training will be provided to ensure confidence in home dialysis processes.

This role emphasizes patient dignity, consistent schedules, and the ability to work independently within a supportive clinical network.

Qualifications

• 1+ year of dialysis experience (home or clinic).
• Willing to complete training for home dialysis setup.
• Strong communication with clinical team and patient.

Responsibilities

• Assist with dialysis treatment setup using NxStage or Tablo.
• Keep the treatment environment clean, organized, and safe.
• Provide companionship and emotional support.
• Help the patient stay on schedule for treatments.
• Provide light ADL support as needed.
• Monitor safety and report concerns to the coordinating nurse.
• Maintain simple documentation and shift notes.
