Job summary

Herewith is seeking a compassionate Care Partner to support a patient transitioning to home dialysis in Mission Viejo, CA. You will work one-on-one with the client, learn patient-specific dialysis routines, and collaborate with our clinical team to ensure a smooth home-based treatment plan.

The role emphasizes dignity, safety, and consistent weekly hours with a single client. Training will be provided to confidently manage home dialysis processes and patient care needs.

Qualifications

• 1+ year of dialysis experience (home or clinic).
• Strong communication with clinical team and patient.
• Willing to complete provided training and work with home dialysis setups.

Responsibilities

• Assist with dialysis treatment setup at home.
• Keep the home treatment area clean and safe.
• Provide companionship and emotional support.
• Help the patient stay on schedule with treatments.
• Monitor safety and report concerns to a nurse.
• Maintain simple shift notes and documentation.
