Job summary

Herewith is seeking a compassionate Care Partner to support a single female dialysis patient returning home in Thousand Oaks, CA. You will be paired with one client, helping with 1:1 care and dialysis setup.

The role emphasizes safe home dialysis environments, companionship, and coordination with the clinical team; prior dialysis experience and NxStage familiarity are required. Start date urgent; weekly hours with a single client.

Qualifications

• 1+ year of experience working with dialysis patients (home or clinic).
• Familiarity with home dialysis equipment and procedures (NxStage a plus).
• Must be reliable, compassionate, and able to communicate with the clinical team.

Responsibilities

• Assist with dialysis treatment setup, (NxStage or similar home machines).
• Ensure the environment is safe, clean, and ready for each treatment.
• Offer comfort, emotional support, and companionship.
• Help the patient stay on schedule with treatment times.
• Support with light Activities of Daily Living (ADLs) as needed.
• Monitor safety and report concerns to the coordinating nurse.
• Maintain simple documentation of observations during your shift.
