Herewith is seeking a compassionate Care Partner to support a female dialysis patient transitioning home in Thousand Oaks, CA. The partner will be paired with one patient, delivering safe home dialysis support and companionship within the patient’s weekly routine.

The role requires dialysis experience, familiarity with home dialysis systems, and a calm, reliable demeanor to respect patient dignity during this transition. Training and a dedicated clinical team are available.

Qualifications

• At least 1 year working with dialysis patients (home or clinic).
• Experience with home dialysis equipment is a plus.

Responsabilités

• Assist with dialysis treatment setup (NxStage or similar home machines).
• Ensure safe, clean environment for each treatment.
• Provide comfort, emotional support, and companionship.
• Help patient stay on schedule with treatment times.
• Assist with light ADLs as needed.
• Monitor safety and report concerns to coordinating nurse.
• Document observations during shift.
