Job summary

Herewith is seeking an experienced In-Home Dialysis Care Partner to support a single female patient transitioning home in Thousand Oaks, CA. You will manage setup, safety, and companionship for home dialysis using NxStage or similar systems, while coordinating with the clinical team.

The role emphasizes reliability, empathy, and clear communication to help the patient stay on schedule and feel secure during this important transition home.

Qualifications

• At least 1 year of experience with dialysis patients (home or clinic).
• Familiarity with home dialysis systems (NxStage a plus).
• Reliable, calm, supportive; understands patient transition.
• Strong communicator who coordinates with the clinical team.
• Must pass a background check through Herewith.

Responsibilities

• Assist with dialysis treatment setup on home machines (NxStage or similar).
• Ensure the environment is safe, clean, and ready for each treatment.
• Offer comfort, emotional support, and companionship.
• Help the patient stay on schedule with treatment times.
• Support light ADLs and monitor safety during shifts.
• Keep simple observations and documentation during the shift.
