Job summary

Froedtert Health in Milwaukee, WI, is seeking a hospital pharmacist to staff on-site, promoting safe, effective medication use and participating in a value-driven clinical team. The role requires independence in decision making, communication with patients and care colleagues, and adherence to Joint Commission/Medicare standards.

A Bachelor's or Pharm.D. from an ACPE-accredited school is required; Wisconsin licensure or eligibility within six months is expected; 1 year hospital experience

Qualifications

• Bachelor's degree in Pharmacy or Pharm.D. from ACPE-accredited school.
• Active Wisconsin Pharmacist License is required or eligible for and must obtain within 6 months.
• Minimum of 1 year hospital experience or 3 years of other pharmacy experience preferred.
• ASHP PGY1 or specialty residency preferred.

Responsibilities

• Promote safe and effective medication use, considering costs and benefits.
• Interact with the healthcare team and patients to proactively address medication therapy issues.
• Ensure patient care complies with the department's quality improvement plan and Joint Commission/Medicare/Medicaid standards.
• Staff varied hours with majority of hours on dayshift; holidays and weekends as described.

Shift Details:

Pharmacist will staff varied hours with majority of hours falling on to dayshiftHolidays: 2 holidays/yearWeekends: every 4th weekend

Job Summary:

Using independent decision making and critical thinking skills, the Pharmacist promotes medication use that is safe, effective and considers the costs and benefits of therapy to maximize value. The Pharmacist interacts with other members of the healthcare team and with patients to proactively address medication therapy issues. The Pharmacist ensures patient care is maintained through compliance with the department's quality improvement plan, and the Joint Commission/Center for Medicare/Medicaid standards for pharmaceutical services.

EXPERIENCE DESCRIPTION:

A minimum of 1 year hospital or 3 years of other pharmacy experience is preferred.

EDUCATION DESCRIPTION:

Bachelor's degree in Pharmacy or Pharm.D. from an ACPE certified school of pharmacy is required. Successful completion of an ASHP accredited PGY1 or specialty residency is preferred.

SPECIAL SKILLS DESCRIPTION:

Demonstrates effective use of computers and applications (e.g. MS Word, Excel, Outlook) as appropriate to practice and communicate effectively. Active membership in the Pharmacy Society of Wisconsin (PSW) preferred. Active membership in the American Society of Health System Pharmacists (ASHP) and/or other national pharmacy organizations as approved by the Pharmacy Administration preferred.

LICENSURE DESCRIPTION:

Active Wisconsin Pharmacist License is required or eligible for and must obtain within 6 months.
