Job summary

The Veterans Health Administration in Salisbury, NC seeks a Registered Nurse (Home and Community Based Care)/Outpatient to manage patient care across inpatient and outpatient settings, coordinate discharge planning, and support education and self-management for Veterans.

The RN will provide competent, evidence-based care, collaborate with Veterans, families, and caregivers, and ensure continuity of care across VAHCBC programs.

Qualifications

• Knowledge of nursing process and patient assessment.
• Ability to coordinate care across inpatient and outpatient settings.
• Experience in discharge planning and transition of care.
• Ability to educate patients and families for self-management.

Responsibilities

• Assess, plan, implement, and evaluate care for patients.
