Job summary

Veterans Health Administration, Department of Veterans Affairs, is seeking a Home Based Primary Care (HBPC) Outpatient Staff Registered Nurse to provide evidence-based care in Veterans’ homes and oversee licensed staff as appropriate. The role centers on case management, care coordination, and comprehensive veteran education within the HBPC program.

The RN will deliver direct veteran care, coordinate with the healthcare team, administer medications, and drive quality improvement and outcomes

Qualifications

• English language proficiency required for direct patient care.
• RN must be licensed and graduate from an approved nursing program.
• Foreign education requires credential evaluation when applicable.
• State licensure must be current and active.

Responsibilities

• Provides home-based primary care and care coordination for HBPC patients.
• Performs assessment, planning, implementation, and evaluation of care in patient homes.
• Delivers medications and procedures per policies; collaborates with interdisciplinary team.
• Leads patient education, self-management support, and care planning for Veterans.
• Assesses age-specific needs and ensures continuity of care across settings.
