Job summary

European Service at Home, Inc. in Rockford, IL is seeking a committed Home Care Aide Supervisor to provide responsive, compassionate client service and direct supervision of Home Care Aides for our senior clients.

The Care Coordinator will work in a team to coordinate care, build client-centered relationships, manage schedules, and maintain accurate records in line with the Care Program. This role offers paid training and robust benefits.

Qualifications

• High School diploma required.
• Excellent communication in English; additional language is a plus.
• Ability to manage Homecare Aides and coordinate care.

Responsibilities

• Work in a team environment to assure the accomplishment of the office's workload and goals.
• Build and foster trusted, client-centered relationships to keep clients engaged in care services based on the Care Program.
• Serve as a resource for technical questions, applications, and problem-solving.
• Evaluate, plan, coordinate, and manage Homecare Aides to ensure the client's and the company's best interests.
• Maintain a good standing relationship with the Community Care Unit.
• Ensure that client and employee records are complete and current.
