Lutheran SeniorLife seeks a Registered Nurse RN - Home Care Coordinator in New Castle, PA, for full-time hours, 8:30 a.m. – 5:00 p.m., with a $6,000 sign-on bonus paid throughout the first year of employment.

The RN will assess home environments and functional capacity, collaborate with the Interdisciplinary Team, determine required home services and time frames, and visit clients in their homes to ensure coordinated care.

Qualifications

• RN license required with licensure to practice nursing in PA.
• BSN preferred; related nursing education and background.
• Minimum of 1 year experience with frail or elderly populations.
• Experience with cognitively impaired seniors is important.
• Able to relate to seniors and families on sensitive issues.
• Must be flexible and able to adapt to change.
• Proficient with computers, Word and Excel.
• Must have dependable transportation and a valid driver’s license.

Overview

Registered Nurse RN - Home Care Coordinator- $6,000 Sign-on Bonus (to be paid throughout the first year of employment).

New Castle, PA

Full-time, 8:30 a.m. to 5:00 p.m.

Lutheran SeniorLife provides a comprehensive array of community-based health and social service programs throughout western Pennsylvania.

Our services include LIFE (Living Independence for the Elderly); skilled nursing, home health; personal care; adoption & foster care; assisted living; adult day services; memory care; hospice; palliative care; residential living; affordable housing; home safety monitoring; Meals on Wheels; rehabilitation services and private duty.

LIFE is a program that serves county residents living in Armstrong, Butler, Beaver and Lawrence Counties. It is an all-inclusive comprehensive program that helps individuals maintain their independence in their home and community through supportive services. The LIFE programs offer an array of person-centered services from medical care to adult day supports with the primary goal of allowing individuals to remain in their own home.

Under the supervision of the Director, Clinic and Home Care Services, the Registered Nurse RN - Home Care Coordinator provides a complete and thorough assessment of the home environment and the functional capabilities of each participant in the program. From this assessment and in collaboration with members of the Interdisciplinary Team, the Registered Nurse RN - Home Care Coordinator must make recommendations for home services. This position is responsible for establishing the services required and the time frames for the services. The Home Care Coordinator must be knowledgeable of the individual participant’s needs.

Qualifications

Registered Nurse RN - Home Care Coordinator requirements include:

• A registered nurse must be a graduate of a school of professional nursing. BSN Preferred.
• Minimum of one (1) years’ experience working with a frail or elderly population.
• Working knowledge of the interdisciplinary model of community care management.
• Experience working with cognitively impaired seniors is important.
• Must be able to relate well to seniors and their families to deal with sensitive issues and facilitate problem solving.
• Must be flexible and able to change easily.
• Working knowledge of computers, Microsoft Word and Excel required.
• Must have dependable transportation with a valid driver’s license in order to visit clients in their homes and other sites.
