Job summary

Community LIFE seeks a Home Care Supervisor in Homestead, PA to provide hands-on leadership and supervision for the home care team. Collaborate with IDT and families to ensure high-quality care.

The role requires 5+ years of direct care experience, 2+ years in supervision, and PA licenses. Weekend/on-call rotation is included, with opportunities to visit participants at home as needed.

Qualifications

• Accredited nursing program for LPN or CNA.
• Program approved by the National Board for Certification in Occupational Therapy.
• PTA program approved by the American Physical Therapy Association.

Responsibilities

Currently offering a $2,500 sign on bonus for this positionLocation: Homestead, PA

Schedule: 7:30a-4p M-F with on call rotation + as needed based on participant care

Summary of Job Duties: The Home Care Supervisor (HCS) will provide consistent supervision to Community Care Aides using effective communication and guidance. This person will strengthen the relationship between the Home Care Department and the Inter Disciplinary Team (IDT). This person will provide hands on leadership in all aspects of home care. The HCS will develop partnerships with families and caregivers. The HCS may be required to make in-home visits as necessary for assessment and on-going case management. This position will participate in weekend/on-call rotation.

Required Education: Graduate of one of the following:

• Accredited nursing program for LPN or CNA.
• Program approved by the National Board for Certification in Occupational Therapy
• Physical Therapy Assistant program approved by the American Physical Therapy Association.

Required Experience: 5 years of direct care experience with at least 1 year of experience working with the frail and elderly population. 2 years of supervisory experience. Experience teaching patients and other health care workers preferred.

Required Certifications/Licensure: Valid Pennsylvania COTA/L, PTA, CNA or LPN license. Valid PA Driver’s License

• Able to deliver services in a compassionate, responsive, and courteous manner.
• Able to work effectively with staff, participants, providers, and referral sources.
• Interest in geriatrics and community-based programming.
