Job summary

Cornerstone VNA is seeking a licensed Speech Therapist (SLP) to join our rehab team in the home setting across New Hampshire and Maine. You will deliver individualized therapy to help clients regain function and communication while supporting independence.

This per diem role involves travel to patients' homes (about 95% travel) with mileage reimbursement and a compassionate, patient-centered approach to care.

Qualifications

• Active NH and ME SLP license or ability to obtain
• At least 1 year of home care experience preferred
• Strong communication and time-management skills
• Passion for making a difference in patients’ lives

Responsibilities

• Evaluate, plan, and implement speech-language pathology treatments in the home setting
• Collaborate with physicians, nurses, and interdisciplinary care teams
• Document progress and adapt care plans based on patient needs
• Educate patients and families to support treatment goals
• Provide compassionate, evidence-based care that promotes dignity and well-being
• Maintain compliance with state and agency requirements
• Accurately document treatments and patient progress in the EMR
• Ensure documentation reflects visit-to-visit progress
