We are currently seeking a passionate and skilled Speech Therapist to join our dynamic Rehab team. You'll provide individualized, patient-centered therapy services that help clients regain function, improve communication, and maintain independence-all while enjoying a healthy work-life balance.
Evaluate, plan, and implement speech-language pathology treatments in the home setting
Collaborate with physicians, nurses, and interdisciplinary care teams
Document progress and adapt care plans based on patient needs
Educate patients and families to support treatment goals
Provide compassionate, evidence-based care that promotes dignity and well-being
Maintain compliance with all state and agency requirements
Accurately document treatments, standardized tests, patient education, and functional progress in the EMR.
Ensure documentation reflects visit-to-visit progress or interventions aimed at preventing deterioration.
Synchronize documentation per agency standards and timelines.
Current SLP license in New Hampshire and Maine (or ability to obtain)
Minimum 1 year of home care experience preferred
Strong communication and time-management skills
