Our pioneering Home Care Connect virtual care model helps patients avoid preventable ER visits and hospitalizations through advanced remote-monitoring tools and 24/7 access to Virtual Care Center RNs.
The Speech Pathologist provides one-to-one, patient-centered care using advanced technology and clinical expertise in the home setting. Care is delivered through a multidisciplinary approach, collaborating with physicians, nursing staff, social workers and home health aides.
Candidates must be highly motivated, detail-oriented, organized, technologically proficient and comfortable working autonomously.
