Job summary

Optum is seeking a Social Worker Field Care Coordinator for the Care at Home program serving DC, MD, and VA. This field-based role requires 50-75% in the member’s home or LTC facility within the greater Washington, D.C. area. Monday–Friday day hours with no on-call or weekend shifts.

The role focuses on individualized care management, care planning across the continuum, and close coordination with an interdisciplinary team to support members in maintaining independence at home.

Qualifications

• Master’s degree in social work or related clinical field.
• Active and unrestricted LICSW or LGSW license in Washington DC or ability to obtain within 90 days.
• 2+ years in long-term care, home health, hospice, public health, or assisted living.
• 1+ years with Electronic Medical Records (EMR).
• 1+ years of clinical case management experience.

Responsibilities

• Assess, plan and implement individualized care management interventions.
• Develop and implement care plans as a single point of contact.
• Communicate health information to stakeholders for coordinated care.
• Advocate for members and families with the care team.
• Document the plan of care in appropriate EHR systems.
