Trinity Health At Home in Georgia seeks a full-time licensed Speech-Language Pathologist (SLP-CCC) to provide in-home therapy for homebound patients in the Athens area. The role includes evaluation, planning, and administering skilled Speech Therapy services as prescribed by a physician.

The SLP will collaborate with an interdisciplinary team to ensure quality care, manage a patient caseload, and maintain open communication.

Qualifications

• 1 year of clinical practice as a licensed Speech Therapist.
• Currently licensed to practice as a Speech Therapist in Georgia.
• Home care or acute care experience required.
• Working knowledge in navigating through Excel and utilization of Microsoft Outlook and Word.
• Currently Valid Driver’s license in the state of Georgia
• CPR certification.
• Regular availability to work.
• Recent home care experience preferred.

The Speech Therapist consults, evaluates, plans and administers skilled Speech Therapy services prescribed by a physician to homebound patientsto restore function, relieve pain and prevent disability following illness, disease, injury, or surgery. Collaborates with Inter-disciplinary team members to assure patient's needs are met, and quality of care is achieved. When acting as the patient care manager, will manage, oversee and provide primary patient care delivery to a select group of patients supportingthe patient centered care model, assuring quality and maintaining open communication. Interacts with any and all members of the care group both internal and external to the organization as needed.

Requirements include :

1 year of clinical practice as a licensed Speech Therapist required.

Currently licensed to practice as a Speech Therapist in Georgia

Home care or acute care experience required

Working knowledge in navigating through Excel and utilization of Microsoft Outlook and Word.

Currently Valid Driver’s license in the state of Georgia

CPR certification.

Regular availability to work.

Recent home care experience preferred.
