Job summary

St. Mary's Home Care & Hospice, an agency of Trinity Health At Home, is seeking a full-time SLP-CCC for a home-care opportunity in the Athens area to provide one-to-one, in-home speech therapy.

The Speech Therapist will consult, evaluate, plan, and administer services, collaborate with the interdisciplinary team, and may act as patient care manager to ensure quality and open communication. Central to this role is compassionate care in the patient's home.

Qualifications

• 1 year of clinical practice as a licensed Speech Therapist in Georgia.
• Current Georgia license to practice Speech Therapy.
• Home care or acute care experience required.
• CPR certification and a valid GA driver's license.
• Proficient use of Excel and Microsoft Outlook/Word.

Responsibilities

• Consult, evaluate, plan and administer skilled Speech Therapy in patients' homes.
• Collaborate with the Inter-disciplinary team to meet patient needs.
• May act as patient care manager to oversee and deliver primary care.
• Maintain open communication with care group members.

The Speech Therapist consults, evaluates, plans and administers skilled Speech Therapy services prescribed by a physician to homebound patientsto restore function, relieve pain and prevent disability following illness, disease, injury, or surgery. Collaborates with Inter-disciplinary team members to assure patient's needs are met, and quality of care is achieved. When acting as the patient care manager, will manage, oversee and provide primary patient care delivery to a select group of patients supportingthe patient centered care model, assuring quality and maintaining open communication. Interacts with any and all members of the care group both internal and external to the organization as needed.

Requirements include

• 1 year of clinical practice as a licensed Speech Therapistrequired.
• Currently licensed to practice as a Speech Therapist in Georgia
• Home care or acute care experience required
• Working knowledge in navigating through Excel and utilization of Microsoft Outlook and Word.
• Currently Valid Driver’s license in the state of Georgia
• CPR certification.
• Regular availability to work.
• Recent home care experience preferred.
