St. Mary's Home Care & Hospice, an agency of Trinity Health, seeks a full-time SLP-CCC for home-based care in the Athens area.

The Speech Therapist will assess, plan and provide skilled services to homebound patients under physician orders, collaborating with an interdisciplinary care team. Requirements include 1 year of clinical practice as a licensed Speech Therapist in Georgia, home care or acute care experience, and proficiency with Excel, Outlook and Word.

Qualifikationen

• 1 year of clinical practice as a licensed Speech Therapist required.
• Currently licensed to practice as a Speech Therapist in Georgia
• Home care or acute care experience required
• Proficient with Excel, Outlook and Word

Aufgaben

• Provide one-to-one, skilled Speech Therapy services prescribed by a physician for homebound patients.
• Collaborate with interdisciplinary team to ensure patient needs are met and quality of care is achieved.
• Act as patient care manager for a select group of patients, overseeing and delivering primary care as needed.
• Maintain open communication with internal and external care teams.
