Job summary

St. Mary’s Home Care & Hospice, an agency of Trinity Health at Home, is seeking a full-time experienced SLP-CCC for a home care opportunity in the Athens area.

The Speech Therapist provides one-to-one, patient-centered care, evaluates and treats homebound patients per physician orders, and collaborates with an interdisciplinary team to ensure quality outcomes. A valid Georgia license and 1 year of clinical practice are required.

Qualifications

• 1 year of clinical practice as a licensed Speech Therapist.
• Currently licensed to practice as a Speech Therapist in Georgia.
• Home care or acute care experience required.
• Working knowledge of Excel, Outlook and Word.
• Valid Georgia driver's license and CPR certification.
• Regular availability to work.
• Recent home care experience preferred.

Responsibilities

• Consult, evaluate, plan and administer skilled Speech Therapy services prescribed by a physician for homebound patients.
• Collaborate with interdisciplinary team to ensure quality of care.
• Act as patient care manager for a select group of patients and oversee primary care delivery.
• Engage with care group members both internal and external to the organization.

It enhances our clinical excellence with advanced, easy-to-use remote monitoring technology and 24/7 access to our Virtual Care Center RNs.

The Speech Therapist consults, evaluates, plans and administers skilled Speech Therapy services prescribed by a physician to homebound patientsto restore function, relieve pain and prevent disability following illness, disease, injury, or surgery. Collaborates with Inter-disciplinary team members to assure patient’s needs are met, and quality of care is achieved. When acting as the patient care manager, will manage, oversee and provide primary patient care delivery to a select group of patients supportingthe patient centered care model, assuring quality and maintaining open communication. Interacts with any and all members of the care group both internal and external to the organization as needed.

Requirements include :

1 year of clinical practice as a licensed Speech Therapistrequired.

Currently licensed to practice as a Speech Therapist in Georgia

Home care or acute care experience required

Working knowledge in navigating through Excel and utilization of Microsoft Outlook and Word.

Currently Valid Driver’s license in the state of Georgia

CPR certification.

Regular availability to work.

Recent home care experience preferred.
