Addus HomeCare, Inc. in Medford, OR is seeking a Service Coordinator to lead scheduling of in-home caregivers, support field staff, and ensure timely updates to care plans.

You will be the problem-solver keeping visits on track in a fast-paced environment. This full-time role requires strong organizational and communication skills, experience in home care, a high school diploma, and a valid driver's license for supervising visits.

Qualifications

• Must have high school diploma or equivalent.
• 3 to 5 years of Industry experience required.
• Interpersonal, organizational and communication skills.
• Computer skills including but not limited to Microsoft Word, Microsoft Excel and Scheduling program.
• Must have DL to complete in home supervisory visits

Responsibilities

• Creates work schedules by entering schedules into the system and adjusts for client needs.
• Provides alternate coverage to ensure care plans are followed.
• Contacts care providers and clients to provide service updates.
• Conducts monthly client wellness calls and home visits as required.
• Supervises direct service employees and enforces policies.
• Maintains confidentiality and a professional branch environment.
• Follows regulations and completes administrative tasks (phones, filing, data entry).
