Job summary

All Metro Health Care seeks a Per Diem RN Supervisor to support our home care clients and caregivers in the Albany & Schenectady area. You will provide clinical oversight, conduct home visits, and mentor Home Health Aides to ensure high-quality, in-home care.

The role emphasizes a flexible, visit-based schedule with supervisory duties, ongoing training, and direct client interaction in patients' homes. Mileage reimbursement is provided.

Qualifications

• Active NYS Registered Nurse license is required.
• Current CPR certification.
• Minimum 1 year of home care experience.
• Minimum 1 year of supervisory or management experience.
• Valid NYS driver’s license, insurance, and reliable vehicle.

Responsibilities

• Provide clinical oversight and mentor Home Health Aides.
• Conduct initial assessments and supervisory home visits.
• Educate and support caregivers on assigned cases.
• Assist physicians in developing and updating plans of care.
• Counsel clients and caregivers regarding nursing and care needs.
• Complete progress notes and report changes in client condition.
• Review clinical records for accurate documentation.
