Essentia Health is seeking a Home Health Registered Nurse to assess, plan, implement, and coordinate patient care in a home setting. You will perform direct nursing care for patients with medical, diagnostic, and emergency needs, under Essentia Health policies and standards.

You will work in various market locations with shift from 08:00 to 16:30, on-call weekends, and possible remote/hybrid option. Current nursing licensure in the state of employment is required along with BCLS certification or

Qualifikationen

• Must hold current nursing licensure in state of employment
• BCLS certification or ability to obtain within 1 month of hire
• Experience in home health or clinical nursing preferred

• Assess, plan, implement, evaluate, supervise and coordinate home care
• Provide direct patient care for medical, diagnostic and emergency conditions
• Coordinate with Essentia Health team to ensure policy adherence and quality care
