Qualifications

• 1:1 home care with pediatric or adult patients.
• Compassionate and skilled RN providing autonomous home visits.
• Strong clinical judgment and patient-first mindset.
• 1 year RN experience preferred; new grads with strong clinical experience encouraged.

Responsibilities

• Develop, implement, and adjust individualized care plans
• Perform skilled nursing interventions based on patient needs and goals
• Administer medications and treatments while monitoring effectiveness
• Assess patient conditions and communicate changes to physicians and care teams
• Provide education to patients and caregivers to support independence
• Coordinate care and assist with discharge planning when appropriate
