Job summary

Entyre Care seeks a detail-oriented Care Manager to perform home visits, train caregivers, and support personalized care plans. You will monitor client health, maintain compliance with standards, and coordinate with physicians and community resources to ensure high-quality in-home care.

The role emphasizes compassion, ownership, and excellence in a startup environment. Hybrid, full-time position offering autonomous care planning and assessment responsibilities.

Qualifications

• RN license required.
• At least 1 year of experience in-home healthcare.
• Experience in case management, care planning and assessment for in-home care.
• Experience supervising caregivers preferred.
• Ability to travel within assigned geographic area; valid driver’s license and auto insurance.

Responsibilities

• Conduct home visits to assess and create personalized care plans for clients and their caregivers.
• Train and support caregivers to ensure they can meet clients’ needs.
• Monitor and report on clients’ health and coordinate with primary care physicians and community resources.
• Facilitate smooth transitions and ongoing care for clients.
• Maintain compliance with relevant regulations and standards.
• Participate in community events to raise awareness and enroll new caregivers.
• Model Entyre Care’s mission, values, and culture.
