Job summary

Homewatch CareGivers of Woodbridge is seeking an experienced RN Care Manager to design person-centered home care plans, mentor caregivers, and champion quality in every family we serve.

In this role, you’ll coordinate with physicians and therapists, monitor client vitals, document in a HIPAA-secure EMR, and lead efforts to keep clients safe, healthy, and comfortable at home while advancing your clinical leadership path.

Qualifications

• Active RN license in NJ.
• Bachelor’s in Nursing or related field, or 2+ years geriatric/home-care experience.
• Strong clinical assessment and problem-solving skills.
• CPR/First Aid certification and valid driver’s license.
• Warm, professional communication with families and field staff.

Responsibilities

• Complete in-home RN assessments and craft individualized care plans with client/family approval.
• Orient caregivers to care plans, perform field competency checks, and ensure ongoing compliance.
• Track vitals, condition changes, and interventions in HIPAA-secure EMR.
• Coordinate with physicians, therapists, and schedulers to maintain seamless, client-centered care.
• Maintain audit-ready client files and uphold state home-care regulations.
