Home Care RN Position Summary

Provides primary nursing care to home-based care patients as ordered by the physician, in accordance with the organization’s policies and procedures. Responsible for initial assessments, care plan development in collaboration with the Population Health Case Manager, and providing functional support to Home Healthcare Aides and LPNs. Focuses on therapeutic intervention, overall care management, and achieving top decile client outcomes and patient satisfaction.

Minimum Qualifications

• Graduate of an approved nursing education program
• Licensed as a Registered Nurse in PA
• At least 1 year of professional nursing experience
• Valid driver’s license and reliable transportation
