Job summary

BJC Home Care, part of BJC HealthCare, is seeking an Intake Coordinator to process referrals and coordinate services in a non-clinical home care setting. This on-site role supports multiple hospital locations and requires strong multitasking and communication with patients and referral sources.

Responsibilities include eligibility screening, insurance verification, and coordinating pre-authorizations and discharge planning to ensure timely and safe patient care.

Qualifications

• High School Diploma or GED is required.
• 2-5 years of related intake/coordinator experience.
• Associate's Degree preferred.
• 5-10 years of experience preferred.
• Supervisory experience not required.

Responsibilities

• Screens referrals to ensure patients meet admission criteria and comply with state and federal regulations.
• Verifies insurance benefits, obtains pre-authorizations as necessary, assists billing, and interprets services and policies to referral sources.
• Tracks current clients that are hospitalized or in short-term facilities or rehab.
• Coordinates discharge planning; educates internal and external customers about Home Care Services and pre-scheduling with service lines.
• Maintains safety as an essential function of the role.
