Job summary

Lehigh Valley Health Network is hiring Home Care Physical Therapists for full-time, part-time and per diem roles across Allentown. The role focuses on evaluating and treating patients at home, developing care plans to restore independence, and coordinating with families.

A $15,000 sign-on bonus plus $5,000 for National Physical Therapy Month is offered for qualifying candidates. Minimum qualifications include a Bachelor's degree in Physical Therapy, PA license, and BLS.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Licensed PT in Pennsylvania on hire.
• BLS certification required (AHA).
• Ability to travel to Home Care assignments.
• Strong clinical reasoning and communication with patients and families.

Responsibilities

• Establishes and implements PT plans of care to restore function.
• Applies exercises, manual techniques, mobility training, and ADL training.
• Communicates patient progress to supervisor and physician(s).
• Completes documentation per policies and regulatory requirements.
• Provides supervision as appropriate within regulatory requirements.
