Job summary

The role requires PA licensure, BLS certification, and the ability to travel to patient care assignments.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Current Pennsylvania PT license upon hire.
• American Heart Association Basic Life Support (BLS) required.
• Ability to travel to patient care assignments - Home Care only.
• Strong clinical reasoning and knowledge of PT techniques.
• Clear and compassionate communication with patients, families, and coworkers.

Responsibilities

• Establishes and implements physical therapy plans of care to restore function and prevent disability.
• Implements individualized treatment using evidence-based techniques such as therapeutic exercises and manual therapy.
• Provides PT treatment aligned with patients' needs assessment and goals.
• Performs reassessments and adjusts plans as needed.
• Communicates patient progress to supervisor, physicians, and team members.
• Delivers care using age- and population-specific approaches; ensures regulatory compliance.
• Maintains documentation and adheres to billing guidelines.
