Qualifications

• Bachelor’s Degree in Physical Therapy required.
• One year of Home Care clinical experience preferred.
• Licensure as Physical Therapist in Pennsylvania on hire or license eligible.

Responsibilities

• Evaluate, plan, direct, and administer physical therapy programs to restore function.
• Develop and update care plans in collaboration with patients and families.
• Perform reassessments and adjust treatment plans as needed.
• Communicate progress with supervising physicians and team members.
• Provide patient education and document clinical interventions per policy.
