Job summary

Metro Physical & Aquatic Therapy is seeking a motivated per diem Physical Therapist to provide in-home care across Western Suffolk County. The position offers flexible scheduling and both 1099 and W-2 options to fit your preferred work style.

Candidate must be a Licensed Physical Therapist in New York or eligible, with home health experience preferred. Training will be provided; travel to patients' homes within Melville/Dix Hills is expected.

Qualifications

• Licensed NY PT or eligible to practice.
• Graduate of an accredited physical therapy program.
• Willingness to travel to patients' homes within geographic area.

Responsibilities

• Assess home PT needs and create treatment plans.
• Develop care plans and goals with physician, patient, and family.
• Implement customized therapy programs to restore function.
• Instruct family members and caregivers on therapy techniques.
• Document visits timely and accurately.
• Communicate discharge plans with patients and team.
• Deliver care per agency policies and standards.

• Compensation: USD 70 - USD 90 - hourly

Responsibilities:

• Accept and treat patients within your designated geographic area
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards
• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

The anticipated pay range for this position is$70-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
