Job summary

Metro Physical & Aquatic Therapy in Eastern Suffolk County is seeking a motivated per diem Physical Therapist to provide in-home care. All levels of experience are welcome and training will be provided; candidates should be licensed in New York or eligible.

You’ll evaluate patients, develop individualized plans, and collaborate with physicians and families. The role offers flexible scheduling, both 1099 and W-2 options, and a supportive, professional team focused on patient care.

Qualifications

• Licensed Physical Therapist in New York or eligible.
• Graduate of a Physical Therapist program.
• Home health experience is preferred but not required.

Responsibilities

• Accept and treat patients within your designated geographic area
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards

• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

Additional Information

The anticipated pay range for this position is $75-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
