Metro Physical & Aquatic Therapy is a comprehensive multidisciplinary company offering Physical, Occupational, Speech Therapy, Massage and Acupuncture services. We seek a motivated per diem Physical Therapist to provide in-home care in Western Suffolk County.

All levels of experience welcome; NY license or eligible. Travel to patients’ homes within the assigned geographic territory is expected. 1099 and W-2 options available; flexible schedule and opportunities for professional growth.

Formación

• Physical Therapist graduate and licensed in NY, or eligible.
• Home health experience preferred but not required; mentor provided.

Responsabilidades

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for home-based physical therapy and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.

• Part-time
• Compensation: USD 70 - USD 90 - hourly

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

The anticipated pay range for this position is$70-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
