Job summary

Responsibilities include evaluating patients, developing treatment plans, and delivering therapy with documentation and teamwork in a supportive environment.

Qualifications

• Licensed Physical Therapist in New Jersey or eligible.
• Graduate from a Physical Therapist program.
• Willingness/ability to travel to patients' homes within geographic territory.
• Home health experience preferred but not required; mentoring provided for documentation/compliance.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.

• Licensed Physical Therapist in the state of New Jersey or eligible.
• Preferred Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
